New Station Enrollment
This online enrollment form includes all of the documents needed by NEW stations wishing to receive an NHOST unit
The package includes:
- Station Welcome Letter from Gordon-Darby NHOST Service
- Online Inspection Fee Form
- Station Agreement and Enrollment Form
- Direct Payment Authorization Form - ACH (Automated Clearing House)
- Station Preparation Information
- High-Speed Access Information
What you will need:
- Your four-digit station ID number (Provided by DMV)
- Your checking account and routing number (For ACH payment form)
- Valid E-mail address for confirmation of your enrollment form

Existing Station Enrollment
This online enrollment form includes all of the documents needed by EXISTING stations required to update their information or station agreement
The package includes:
- Station Agreement and Enrollment Form
What you will need:
- Your four-digit station ID number (Provided by DMV)
- Your checking account and routing number (For ACH payment form only if updating)
- Valid E-mail address for confirmation of your enrollment form
Required fields are marked with a red asterisk and will need to be filled out completely.
This package will be completed and submitted online. You will need to provide a valid e-mail address to complete the form.
ACH Update Form
This form is only to make changes to the checking account on file with NHOST Services for billing purposes.
What you will need:
- Your four-digit station ID number (Provided by DMV)
- Your checking account and routing number (For ACH payment form)
- Valid E-mail address for confirmation of your ACH form
This form will be completed and submitted online. You will need to provide a valid e-mail address to complete the form.
Manager Access Permission Form
This form is only to modify the permission levels of managers.
What you will need:
- Your four-digit station ID number (Provided by DMV)
- The NH Driver's License number of all Non-Inspectors you are enrolling
- Valid E-mail address for confirmation of your Manager Access Permission form
This form will be completed and submitted online. You will need to provide a valid e-mail address to complete the form.
